General Occupational Therapist required for Front Door Team. JD available on request: To provide, develop and deliver rapid assessment and timely discharges within the Emergency Department and Acute Assessment units. Using specialist and skilled levels of Physiotherapy/Occupational Therapy intervention to work collaboratively with community services and the MDT to prevent hospital admission and reduce readmission rates. Working within an integrated OT/PT service with shared skills to facilitate seamless working and the ability to flex to demand, across a seven-day service. Working hours: shifts, 09.00 – 16.00.
