Are you an experienced Senior Occupational Therapist looking for your next locum opportunity within the NHS? Mid and South Essex NHS Foundation Trust is seeking a highly motivated and enthusiastic Band 6 Occupational Therapist to join its Discharge to Assess Service, providing reablement and rehabilitation support to patients across Mid Essex.

This role offers the opportunity to help adults regain independence following illness, injury or a period of hospital care, supporting recovery within the familiar surroundings of their own homes. You will work collaboratively with physiotherapists, occupational therapists, rehabilitation assistants and domiciliary care staff to deliver person-centred rehabilitation and reablement interventions.

You will undertake community-based assessments, establish meaningful rehabilitation goals and coordinate appropriate care and equipment to help patients maximise their independence, maintain safety and avoid unnecessary hospital admissions.

An immediate start is required, with an ongoing locum contract available for a suitably experienced Occupational Therapist.

Key Responsibilities

  • Undertake comprehensive occupational therapy assessments in patients’ own homes to identify functional needs, risks and opportunities for rehabilitation.

  • Develop, implement and evaluate person-centred rehabilitation programmes and reablement goals tailored to individual needs.

  • Provide ongoing rehabilitation and reablement interventions to help patients regain or maintain independence with everyday activities.

  • Support patients with urgent care needs in the community, helping to prevent avoidable hospital admissions where appropriate.

  • Assess patients’ functional abilities, mobility, transfers and ability to manage activities of daily living safely.

  • Identify and assess equipment requirements to promote independence, improve safety and support effective care at home.

  • Coordinate appropriate care and rehabilitation services, working closely with patients, families and relevant professionals.

  • Collaborate with physiotherapists, Occupational Therapists, rehabilitation assistants and domiciliary care staff to deliver an integrated service.

  • Monitor patients’ progress, review rehabilitation goals and adapt interventions in response to changing needs.

  • Contribute to effective discharge planning and support safe transitions from hospital to community-based care.

  • Maintain accurate clinical records and ensure assessments and interventions comply with relevant professional standards and service requirements.

About You

You will be a confident and experienced Senior Occupational Therapist with a strong interest in community rehabilitation, reablement and supporting patients to remain independent in their own homes.

You will have the clinical reasoning skills to assess complex functional needs, identify risks and develop practical rehabilitation plans that deliver meaningful outcomes. You will also be comfortable working independently in the community while collaborating closely with a multidisciplinary team.

Your ability to communicate effectively, coordinate services and respond flexibly to changing patient needs will be essential in helping the service deliver safe, effective and timely care.

Requirements

  • A recognised Occupational Therapy qualification.

  • Current registration with the Health and Care Professions Council (HCPC).

  • Relevant post-qualification experience in occupational therapy, ideally within community rehabilitation, reablement or discharge services.

  • Experience undertaking functional assessments in patients’ own homes.

  • Knowledge of rehabilitation approaches, goal setting and interventions to promote independence in activities of daily living.

  • Experience assessing equipment needs and identifying risks within the home environment.

  • Ability to manage a caseload, prioritise referrals and respond to changing clinical needs.

  • Strong communication and multidisciplinary working skills.

  • Ability to work autonomously in the community while contributing effectively to a wider rehabilitation team.

  • Availability to start as soon as possible.
