Occupational Therapist – Hospital Discharge Team

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity within North London Social Services. Based with the London Borough of Haringey, this role sits within the Adult Social Care Hospital Discharge Team, supporting safe, timely, and effective hospital discharges through the Care Act 2014 and Discharge to Assess (D2A) model.

This is an excellent opportunity to join a fast-paced multidisciplinary team, helping adults transition safely from hospital to home or the most appropriate care setting. You will undertake specialist Occupational Therapy assessments, identify rehabilitation and equipment needs, and promote strengths-based interventions that maximise independence while reducing unnecessary hospital stays and readmissions.

Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments in line with the Care Act 2014.
  • Complete strengths-based assessments focused on independence, wellbeing, and prevention.
  • Assess functional ability and identify rehabilitation, equipment, assistive technology, and adaptation needs.
  • Carry out moving and handling assessments and recommend appropriate interventions.
  • Develop person-centred, outcome-focused support plans that promote independence and reduce reliance on long-term care.
  • Facilitate safe and timely hospital discharges through the Discharge to Assess (D2A) pathway.
  • Work collaboratively with hospital teams, social workers, therapists, nurses, and community providers to coordinate discharge planning.
  • Participate in multidisciplinary meetings and contribute specialist Occupational Therapy expertise.
  • Provide Occupational Therapy duty cover, responding to urgent referrals and same-day discharge assessments.
  • Support hospital flow by prioritising urgent cases and helping to reduce delayed discharges.
  • Provide professional advice and guidance to colleagues on complex functional assessments, moving and handling, and risk management.
  • Maintain accurate, timely, and legally compliant clinical records and documentation.

About You

You will be an experienced HCPC-registered Occupational Therapist with a strong background in hospital discharge, adult social care, or community rehabilitation. You will be confident working within fast-paced multidisciplinary environments and have excellent clinical reasoning skills to support complex discharge planning and promote positive outcomes.

You will be passionate about strengths-based practice, enabling people to regain independence while ensuring safe and effective transitions from hospital to home.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care, Hospital Discharge, or Community Occupational Therapy.
  • Strong knowledge of the Care Act 2014 and Discharge to Assess (D2A) model.
  • Experience undertaking functional, moving and handling, and equipment assessments.
  • Knowledge of assistive technology and housing adaptations.
  • Experience working within multidisciplinary teams to facilitate safe hospital discharges.
  • Excellent assessment, clinical reasoning, communication, and report-writing skills.
  • Ability to prioritise urgent referrals and manage a fast-paced caseload.
