Monmouthshire County Council is seeking an experienced Occupational Therapist to join its Integrated Services Team on an ongoing part-time locum basis.

The successful candidate will deliver high-quality Occupational Therapy services across people’s homes and community settings, supporting individuals to maintain independence, avoid unnecessary hospital admissions and access appropriate care, equipment and adaptations.

This is a community-based role requiring regular travel to visit service users in their homes. The successful candidate will need to demonstrate a flexible and responsive approach, particularly when supporting urgent needs and the service’s Duty function.

Experience working alongside Reablement Services is highly relevant. Welsh language skills are desirable but not essential.

Key Responsibilities

• Deliver specialist Occupational Therapy interventions within people’s homes and community settings to promote independence and prevent unnecessary hospital admissions.

• Work collaboratively with the Reablement Service to maximise individuals’ independence and functional outcomes.

• Complete comprehensive Occupational Therapy assessments for individuals with long-term and complex needs.

• Develop and implement tailored, person-centred intervention plans.

• Provide timely therapeutic input to support admission avoidance and safe community living.

• Assess, recommend and arrange appropriate assistive equipment and home adaptations.

• Process and support applications for Disabled Facilities Grants (DFGs).

• Undertake housing assessments and contribute to manual handling and reablement plans.

• Participate in a weekly Duty rota, responding to queries and undertaking urgent community visits.

• Act as a Trusted Assessor, establishing care packages based on assessed needs and reviewing existing care arrangements.

• Manage a varied caseload effectively, prioritising work according to clinical need and urgency.

• Maintain accurate professional records and ensure all interventions comply with relevant professional and organisational standards.

• Work collaboratively with service users, families, carers, health and social care professionals, housing teams, care providers and equipment suppliers.

• Community-based service users, families and carers.

• Multidisciplinary teams within Chepstow Community Hospital.

• Social Care teams and Housing departments.

• External agencies and partner organisations.

• Duty team colleagues and the wider Occupational Therapy service.

• Care providers and equipment suppliers.

Essential Skills & Experience

• Degree or equivalent qualification in Occupational Therapy.

• Current HCPC registration.

• Experience working within a community, social care or integrated health setting.

• Strong assessment and clinical reasoning skills.

• Experience supporting individuals with long-term and complex needs.

• Knowledge of equipment, adaptations and housing assessments.

• Experience with Disabled Facilities Grants (DFGs) is desirable.

• Understanding of manual handling, reablement and admission avoidance.

• Experience reviewing or setting up care packages is desirable.

• Ability to work autonomously and manage a varied community caseload.

• Excellent organisational, communication and multidisciplinary working skills.

• Ability to travel throughout the community to undertake home visits.

• Welsh language skills are desirable but not essential.
