  • Work collaboratively with housing teams, contractors, social workers, and healthcare professionals to facilitate timely adaptations.
  • Manage a varied community caseload, ensuring assessments and interventions are completed efficiently.
  • Maintain accurate, timely, and professional case records in line with HCPC and organisational standards.
  • Promote strengths-based, person-centred practice that supports individuals to remain independent in their own homes.

About You

You will be an experienced Occupational Therapist with a strong background in Adult Social Care and community-based practice. You will be confident assessing for major and minor adaptations, recommending specialist equipment, and working collaboratively with multidisciplinary teams to achieve the best possible outcomes for adults.

You will possess excellent assessment, communication, and clinical reasoning skills, together with a commitment to delivering high-quality, person-centred Occupational Therapy services.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care or Community Occupational Therapy.
  • Experience assessing for major and minor housing adaptations.
  • Knowledge of specialist equipment and assistive technology provision.
  • Understanding of moving and handling principles and risk assessment.
  • Excellent assessment, report-writing, and communication skills.
  • Ability to manage an independent community caseload.

Contract & Salary
