Occupational Therapist

Brief Job Overview

Tripod Partners are recruiting an Occupational Therapist for a locum role within 
South West London Social Services. This position sits across the hospital team and the 
reablement service, supporting individuals to regain independence and transition safely from hospital 
back into the community.

Working within adult social care, the Occupational Therapist will carry out functional assessments and 
contribute to discharge planning for individuals requiring support following hospital admission. The role 
involves close collaboration with health and social care professionals to ensure people receive the right 
interventions, equipment, or support to live safely and independently after leaving hospital.

Key Responsibilities

  • Carry out occupational therapy assessments for individuals within hospital and reablement services.
  • Support discharge planning by identifying needs related to daily living and independence.
  • Work collaboratively with multidisciplinary teams to support safe and timely hospital discharge.
  • Develop person-centred plans that support recovery and independence following hospital admission.
  • Recommend appropriate equipment, adaptations, or reablement interventions where required.
  • Maintain accurate clinical documentation and case records in line with professional standards.
  • Contribute to multidisciplinary discussions regarding care planning and rehabilitation outcomes.

About You

You will be a qualified Occupational Therapist with experience working within hospital 
discharge services, reablement teams, or adult social care settings. You will be confident assessing 
functional needs and recommending practical interventions that enable individuals to live safely and 
independently in their own homes.

Strong communication and collaborative working skills are important in this role, particularly when 
working with healthcare professionals and social care teams to support effective discharge planning.

Requirements

  • Recognised qualification in Occupational Therapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience working within hospital discharge, reablement, or adult social care services.
  • Ability to assess functional needs and recommend appropriate equipment or interventions.
  • Strong multidisciplinary working and communication skills.
