- Report to an Occupational Therapist within Reablement
- Carry out strengths-based occupational therapy assessments and reviews for individuals receiving occupational therapy equipment and domiciliary care
- Undertake assessments under relevant legislation, including the Care Act and the Housing Grants, Construction and Regeneration Act
- Reduce handoffs and ensure environmental changes that support independence are reflected in care provision
- Work in partnership with service users, carers, and a range of professionals to promote independence and wellbeing
- Deliver occupational therapy interventions that support independence
- Accurately record contact and case notes on appropriate IT systems
