Reablement Assistant

Brief Job Overview

Tripod Partners are recruiting an experienced Reablement Assistant for a locum opportunity within Hampshire Social Services. Based with Southampton City Council, this community-based role supports adults to regain independence and confidence following hospital discharge or changes in their health, helping them to remain safely within their own homes.

Working as part of a multidisciplinary reablement service, you will undertake strengths-based assessments, develop person-centred reablement plans, coordinate short-term interventions, and work closely with health and social care partners to deliver positive outcomes. This is an excellent opportunity for an experienced Senior Carer or Team Leader looking to further develop their assessment and care coordination skills within Adult Social Care.

Key Responsibilities

  • Undertake strengths-based assessments and reviews to identify individual needs, goals, and desired outcomes.
  • Develop and coordinate personalised reablement plans that promote independence, recovery, and wellbeing.
  • Support individuals following hospital discharge or within the community to maximise their functional abilities.
  • Provide advice, guidance, and signposting to statutory, voluntary, and community services.
  • Assess, manage, and monitor risk while applying Making Safeguarding Personal principles.
  • Work collaboratively with Occupational Therapists, Physiotherapists, Social Workers, community health teams, and other partner agencies.
  • Support safe and timely hospital discharge, including coordinating equipment provision and onward referrals.
  • Maintain accurate, timely, and legally compliant case records and documentation.
  • Provide guidance, mentoring, and role modelling of best practice to frontline staff.
  • Contribute to service development, quality improvement, and performance objectives.

About You

You will be an experienced care professional with strong assessment skills and a passion for supporting adults to regain independence. You will be confident managing a varied caseload, working autonomously, and building effective relationships with service users, carers, and multidisciplinary partners.

You will enjoy working in a fast-paced community environment and be committed to delivering person-centred, strengths-based care that enables individuals to achieve the best possible outcomes.

Requirements

  • Minimum Level 3 Diploma/NVQ in Adult Social Care (or equivalent).
  • Experience working as a Senior Carer, Team Leader, or in a similar role within health or Adult Social Care.
  • Demonstrable assessment and care planning experience.
  • Experience within reablement, rehabilitation, or short-term intervention pathways is highly desirable.
  • Enhanced DBS (Adult Workforce).
  • Knowledge of strengths-based practice and person-centred care.
  • Understanding of safeguarding principles, including Making Safeguarding Personal.
  • Excellent communication, organisational, and problem-solving skills.
  • Ability to work independently and collaboratively within multidisciplinary teams.
