Community Occupational Therapist – Leeds City Council, West Yorkshire

 

Job Overview

We are looking for a dedicated and professional Community Occupational Therapist to join Leeds City Council’s Adult Social Care team in West Yorkshire on a locum basis. This role involves delivering high-quality occupational therapy services within a community care bed setting, supporting adults to regain independence and return home safely.  

You will work as part of a multidisciplinary team to assess, plan, and implement evidence-based interventions that reduce barriers to daily living and improve wellbeing. The position may include weekend work on a rota basis. Previous experience in an intermediate care or reablement setting is desirable.
 

Key Responsibilities

 

  •  
    • Undertake a range of complex occupational therapy assessments (including reablement assessments) to identify needs and support independence.
       

    • Take responsibility for the care planning process, from assessment through to intervention and review.
       

    • Manage a caseload effectively, monitoring and evaluating progress to ensure interventions remain person-centred and outcome-focused.
       

    • Carry out moving and handling assessments and provide training, guidance, and specialist recommendations for staff, carers, and service users.
       

    • Recommend and facilitate the provision of specialist equipment, aids, and adaptations to promote safety and mobility.
       

    • Work within the legislative framework to make recommendations for complex adaptations that reduce environmental barriers and enhance independence.
       

    • Undertake enhanced screening of referrals and contribute to decision-making regarding service provision and resource allocation.
       

    • Deliver and review outcome-focused OT interventions aimed at enabling independence, inclusion, and wellbeing.
       

    • Assess specialist housing needs and support residents to access appropriate housing or home adaptations.
       

    • Ensure accurate and timely documentation, maintaining case notes, risk assessments, and reports in accordance with professional standards.
       

    • Participate in safeguarding investigations in line with legislative and multi-agency procedures.
       

    • Work collaboratively with health, housing, and social care colleagues to ensure coordinated and effective support.
       

    • Comply with the College of Occupational Therapists’ standards and the policies and practices of Leeds City Council.

     

    About You

    You are a confident, self-motivated Occupational Therapist who is committed to supporting individuals in achieving independence and improving quality of life. You combine strong clinical reasoning with empathy and professionalism, and thrive in fast-paced, multidisciplinary environments. Experience in intermediate care or community-based rehabilitation will be advantageous.
     

    Requirements

     

    •  
      • Degree or Diploma in Occupational Therapy with current HCPC registration.
         

      • Previous experience in community, intermediate care, or reablement settings is desirable.
         

      • Knowledge of equipment provision, housing adaptations, and moving and handling techniques.
         

      • Ability to manage a varied caseload and deliver effective, outcome-focused interventions.
         

      • Excellent written and verbal communication skills, including report writing and record keeping.
         

      • Commitment to safeguarding principles and professional standards of practice.

       

      Contract & Salary  

        • Position: Locum Community Occupational Therapist
           

        • Location: Leeds City Council, West Yorkshire
           

        • Pay rate: £24.98 – £33.97 per hour (Umbrella)
           

        • Hours: Full-time, 37 hours per week (including potential weekend rota work)
           

        • Contract length: Ongoing
