Occupational Therapist – Band 6

 

Brief Job Overview

 

Tripod Partners are recruiting a Band 6 Occupational Therapist for a locum role within 
East Riding of Yorkshire NHS. This position focuses on providing occupational therapy 
support within Intermediate Care services, helping individuals regain independence and 
transition safely from hospital to home or community settings.

 

Working within an intermediate care team, the Occupational Therapist will assess functional abilities, 
support discharge planning, and contribute to rehabilitation programmes that enable individuals to 
maintain independence following illness or hospital admission. The role may include visits to patients’ 
homes and other service locations as part of community-based rehabilitation and discharge support.

 

Key Responsibilities

 

  • Carry out occupational therapy assessments for individuals within intermediate care services.
  • Support hospital discharge and discharge-to-assess pathways where appropriate.
  • Develop person-centred rehabilitation plans that promote independence and safe daily living.
  • Work collaboratively with multidisciplinary teams supporting intermediate care and rehabilitation services.
  • Recommend appropriate equipment or adaptations to support individuals returning home.
  • Maintain accurate clinical documentation in line with NHS standards.
  • Provide guidance and support to patients and carers to aid recovery and independent living.

 

About You

 

You will be a qualified Occupational Therapist with experience working within intermediate 
care, rehabilitation, or hospital discharge services. You will be confident assessing functional needs 
and supporting individuals as they transition from hospital-based care to community living.

 

Strong communication and multidisciplinary teamwork skills are important, as the role requires close 
collaboration with healthcare professionals to support safe discharge planning and rehabilitation 
outcomes.

 

Requirements

 

  • Recognised Occupational Therapy degree.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience working within intermediate care services.
  • Experience supporting hospital discharge, discharge-to-assess, or rehabilitation pathways desirable.
  • Experience using SystmOne desirable but not essential.
  • Full driving licence and access to a car required due to potential travel to other sites and patient homes.
