Service: Independent Living Service – Community OT Team (Pathway 1 Reablement)

 

Job Details:

 

  • Senior Occupational Therapist required to support the Community OT Team within the Independent Living Service.
  • Focus on Pathway 1 Reablement, supporting individuals to regain independence following hospital discharge or illness.
  • Carry out occupational therapy assessments and interventions within community settings.
  • Develop and implement reablement plans to maximise independence and functional ability.
  • Work collaboratively with multidisciplinary teams to support safe discharge and ongoing care.
  • Provide professional expertise and guidance within the team as a senior practitioner.
  • Manage a caseload of individuals with varying levels of complexity.

 

Skills / Experience:

 

  • Qualified Occupational Therapist with HCPC registration.
  • Experience working in adult social care and community settings.
  • Experience in reablement and discharge pathways.
  • Ability to work autonomously and manage a caseload effectively.
  • Strong communication and multidisciplinary working skills.

 

Working Hours: Part time – 3 days per week.
