Physiotherapist – Band 4

Tripod Partners are recruiting a Band 4 Physiotherapist (Associate Practitioner) for a locum role within North West NHS. This position sits within the Transfer of Care Hub, supporting community services that help individuals transition safely from hospital to home.
Working as part of a multidisciplinary team, the Physiotherapy Associate Practitioner will support rehabilitation and discharge pathways by assisting with therapy interventions and contributing to the delivery of community-based rehabilitation services. The role focuses on supporting patients following hospital admission, helping to promote independence and reduce the risk of readmission through coordinated rehabilitation support.
Key Responsibilities
- Support physiotherapy and rehabilitation interventions within the Transfer of Care Hub.
- Assist with rehabilitation programmes designed to improve mobility and independence.
- Work collaboratively with physiotherapists and multidisciplinary team members within community services.
- Support individuals transitioning from hospital-based care into the community.
- Contribute to patient-centred care plans and rehabilitation pathways.
- Maintain accurate records and documentation in line with NHS standards.
About You
You will be a motivated healthcare professional with experience supporting rehabilitation or therapy services. You will be comfortable working as part of a multidisciplinary team and assisting with physiotherapy interventions aimed at improving mobility and independence.
Strong communication skills and the ability to work within community services will support success in this role, particularly when supporting individuals following hospital discharge.
Requirements
- Experience supporting physiotherapy or rehabilitation services.
- Ability to work effectively within multidisciplinary healthcare teams.
- Strong communication and organisational skills.
- Understanding of community rehabilitation or discharge support pathways desirable.
