Service: Independent Living Service – Community OT Team (Pathway 1 Reablement)
Job Details:
- Senior Occupational Therapist required to support the Community OT Team within the Independent Living Service.
- Focus on Pathway 1 Reablement, supporting individuals to regain independence following hospital discharge or illness.
- Carry out occupational therapy assessments and interventions within community settings.
- Develop and implement reablement plans to maximise independence and functional ability.
- Work collaboratively with multidisciplinary teams to support safe discharge and ongoing care.
- Provide professional expertise and guidance within the team as a senior practitioner.
- Manage a caseload of individuals with varying levels of complexity.
Skills / Experience:
- Qualified Occupational Therapist with HCPC registration.
- Experience working in adult social care and community settings.
- Experience in reablement and discharge pathways.
- Ability to work autonomously and manage a caseload effectively.
- Strong communication and multidisciplinary working skills.
Working Hours: Part time – 3 days per week.
