Job Overview

Oxfordshire County Council is seeking an experienced Occupational Therapist to join its Home First Team, supporting the transformation of adult social care and hospital discharge services across Oxfordshire.

The role focuses on assessing individuals in their own homes following hospital discharge, supporting safe and timely discharge through the Discharge to Assess pathway, and helping people regain or maintain independence.

The successful candidate will work collaboratively with multidisciplinary partners to provide personalised interventions, equipment and reablement-focused solutions that reduce reliance on ongoing care where appropriate.

Key Responsibilities

  • Assess individuals in their own homes following hospital discharge.

  • Support safe and timely hospital discharge through the Discharge to Assess pathway.

  • Complete comprehensive functional assessments and activity analysis.

  • Assess, recommend and prescribe appropriate equipment, assistive technology and minor adaptations.

  • Undertake moving and handling assessments and reviews.

  • Develop personalised support plans in partnership with individuals, families, carers and multidisciplinary professionals.

  • Identify practical interventions that promote independence and reduce reliance on ongoing care.

  • Support individuals and their families through periods of change, rehabilitation and recovery.

  • Provide professional Occupational Therapy advice and recommendations to partner agencies.

  • Work collaboratively with health, social care and community professionals to achieve positive outcomes.

  • Apply clinical reasoning and problem-solving skills to complex functional situations.

  • Maintain accurate, timely and professionally defensible case records.

  • Manage a caseload effectively while prioritising individuals according to need and risk.

  • Promote strengths-based and person-centred practice throughout all assessments and interventions.

  • Travel across Oxfordshire as required to complete home visits and assessments.

Skills & Experience

  • Degree or equivalent qualification in Occupational Therapy.

  • Current HCPC registration.

  • Experience working as an Occupational Therapist within community rehabilitation, Adult Social Care, hospital discharge or a similar setting.

  • Strong assessment, problem-solving and clinical reasoning skills.

  • Experience with equipment provision, moving and handling and community rehabilitation.

  • Understanding of hospital discharge and Discharge to Assess pathways.

  • Ability to assess functional needs within individuals’ home environments.

  • Strong commitment to promoting independence and strengths-based practice.

  • Ability to work autonomously while contributing effectively to a multidisciplinary team.

  • Excellent communication and organisational skills.

  • Ability to develop positive working relationships with individuals, families, carers and partner agencies.

  • Willingness and ability to travel across Oxfordshire as required.

  • Commitment to the principles of the Oxfordshire Way.
