Occupational Therapist (Part Time, Locum)

Job Overview

We are seeking an experienced Occupational Therapist to join Monmouthshire County Council, based at Chepstow Community Hospital, within the South East Wales Social Services area. This part-time locum role is initially for 12 weeks and involves delivering high-quality occupational therapy interventions both within the community hospital and across people’s homes. You will support individuals with a range of long-term and complex needs, promoting independence, preventing unnecessary hospital admissions, and ensuring access to appropriate care, equipment, and adaptations.

 

Key Responsibilities

  •   
    • Deliver occupational therapy assessments and interventions in patients’ homes and community settings, supporting independence and wellbeing.
       

    • Conduct comprehensive assessments and develop personalised intervention plans for individuals with long-term, complex needs.
       

    • Support admission avoidance through timely, evidence-based therapeutic input.
       

    • Recommend and arrange assistive equipment and home adaptations, including involvement in Disabled Facilities Grants (DFGs).
       

    • Carry out housing assessments and contribute to manual handling plans and reablement programmes.
       

    • Act as a Trusted Assessor to set up care packages based on identified needs and review existing care arrangements.
       

    • Participate in the weekly Duty rota (one day per week), responding to urgent queries and conducting timely visits when needed.
       

    • Manage a varied caseload effectively, demonstrating strong time management, organisational ability, and clinical reasoning.

     

    About You

     

    You are a confident and skilled Occupational Therapist with experience working in community settings or hospital-based roles that extend into the community. You bring strong assessment skills, sound clinical judgement, and the ability to work autonomously while maintaining collaborative relationships with service users, carers, and multidisciplinary colleagues. Your commitment to promoting independence and preventing avoidable admissions will be central to your success in this role.

    Requirements
     

    • Degree or diploma in Occupational Therapy and registration with the Health and Care Professions Council (HCPC).
       

    • Experience working in community or mixed hospital/community roles.
       

    • Strong assessment, intervention planning, and documentation skills.
       

    • Knowledge of equipment, housing adaptations, and DFG processes.
       

    • Experience contributing to reablement, manual handling plans, and multidisciplinary working.
       

    • Ability to work independently and manage a varied part-time caseload.
