Role Overview
We are seeking an experienced Band 6 Community Physiotherapist to join the team at Broomfield Hospital. This role involves delivering high-quality assessment, treatment and rehabilitation to adults in the community, supporting safe discharge and promoting independence.
Key Responsibilities
- Provide physiotherapy assessments and intervention in community settings.
- Support patients with rehabilitation, mobility, and functional independence.
- Develop and implement personalised treatment plans.
- Work collaboratively with multidisciplinary teams and external services.
- Contribute to safe discharge planning and reablement goals.
- Maintain accurate and timely clinical documentation.
About You
- HCPC-registered Physiotherapist.
- Previous community or discharge pathway experience desirable.
- Strong clinical reasoning and assessment skills.
- Confident working independently and as part of an MDT.
- Excellent communication and organisational abilities.
