Brief Job Overview

 

Tripod Partners are recruiting an Occupational Therapist for a locum role within South East Wales Social Services. This position sits within an Integrated Services Team, delivering community-based occupational therapy to support individuals to remain independent and reduce hospital admissions.

 

The role focuses on providing high-quality, person-centred interventions within people’s homes and community settings. You will work closely with reablement services and multidisciplinary teams to deliver timely assessments, equipment provision, and support planning for individuals with complex needs.

 

Key Responsibilities

 

  • Deliver occupational therapy assessments and interventions within home and community settings.
  • Support independence and prevent unnecessary hospital admissions.
  • Develop tailored intervention plans for individuals with complex and long-term needs.
  • Work alongside reablement services to support recovery and functional outcomes.
  • Recommend and arrange assistive equipment and home adaptations, including Disabled Facilities Grants (DFG).
  • Undertake housing and manual handling assessments.
  • Participate in a Duty rota, responding to urgent referrals and queries.
  • Act as a Trusted Assessor, setting up and reviewing care packages.
  • Manage a varied caseload effectively, prioritising urgent and complex cases.
  • Work collaboratively with multidisciplinary teams, social care, housing, and external partners.
  • Maintain accurate and timely records in line with statutory and professional standards.

 

About You

 

You will be a qualified Occupational Therapist with experience working within 
community or integrated social care services. You will demonstrate strong clinical 
reasoning skills and the ability to manage a varied caseload independently.

 

You will be flexible and responsive, with the ability to work both autonomously and collaboratively to 
deliver effective, person-centred outcomes.

 

Requirements

 

  • Recognised qualification in Occupational Therapy.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience within community or integrated services.
  • Experience in equipment provision, adaptations, and reablement.
  • Ability to undertake manual handling and housing assessments.
  • Strong communication, organisational, and problem-solving skills.
  • Ability to manage a varied caseload and respond to urgent needs.
  • Welsh language skills desirable but not essential.
