Specialist Physiotherapist – Band 6

Brief Job Overview

Tripod Partners are recruiting an experienced Band 6 Specialist Physiotherapist for a locum opportunity with Sussex Community NHS Foundation Trust, based at Princess Royal Hospital. Working within the Sussex NHS Hospital Rapid Discharge Team, you will play a vital role in facilitating safe and timely hospital discharges, supporting patients to transition successfully from hospital to community settings.

This is an excellent opportunity for a Physiotherapist with experience in acute, community, discharge planning, or intermediate care services. Working as part of a multidisciplinary team, you will assess patients’ rehabilitation needs, promote functional independence, and contribute to reducing delayed discharges while helping to prevent unnecessary hospital readmissions.

Key Responsibilities

• Provide specialist physiotherapy assessment and intervention for patients requiring rapid discharge from hospital.
• Assess mobility, balance, strength, transfers, gait, and functional ability to support safe discharge planning.
• Develop and implement person-centred rehabilitation programmes that maximise independence and recovery.
• Work collaboratively with multidisciplinary colleagues to coordinate timely discharge pathways and ongoing rehabilitation.
• Provide rehabilitation advice, exercise programmes, and education to patients, carers, and families.
• Recommend appropriate mobility aids, equipment, and rehabilitation strategies to support safe discharge.
• Identify and manage falls risks while promoting safe mobility within home and community environments.
• Support early discharge initiatives and contribute to reducing unnecessary hospital stays and readmissions.
• Maintain accurate clinical documentation and ensure effective communication with healthcare professionals and community services.
• Deliver evidence-based Physiotherapy practice in accordance with HCPC standards, Trust policies, and professional guidelines.

About You

You will be a HCPC-registered Physiotherapist with experience working within acute hospitals, community rehabilitation, intermediate care, or discharge planning services. You will have excellent assessment, rehabilitation, and clinical reasoning skills together with a patient-centred approach to supporting individuals with complex rehabilitation needs.

You will thrive in a fast-paced multidisciplinary environment, demonstrating excellent communication, organisational skills, and the ability to manage competing priorities while delivering high-quality patient care.

Requirements

• Degree or Diploma in Physiotherapy.
• Current HCPC registration.
• NHS experience as a Physiotherapist.
• Experience within acute care, community rehabilitation, intermediate care, or hospital discharge services is highly desirable.
• Strong assessment and rehabilitation skills relating to mobility, balance, strength, and functional independence.
• Excellent communication, clinical reasoning, and multidisciplinary team-working skills.
• Ability to work effectively within a fast-paced hospital environment.
• Commitment to delivering high-quality, person-centred rehabilitation.
