Brief Job Overview

Tripod Partners are recruiting an experienced Reablement Physiotherapist for a locum opportunity with the London Borough of Newham. Working within East London Social Services, you will play a key role in delivering community-based reablement services that enable adults to regain independence, improve mobility, and remain safely in their own homes following illness, injury, or hospital discharge.

Working closely with Reablement Workers, Occupational Therapists, Social Workers, and other health professionals, you will provide specialist physiotherapy assessment and rehabilitation to support recovery, prevent unnecessary admissions to hospital, and maximise functional independence. This hybrid role includes one office-based day per week alongside community visits across the borough.

Key Responsibilities

• Undertake comprehensive physiotherapy assessments for adults receiving community reablement services.
• Develop and deliver personalised rehabilitation programmes that improve mobility, strength, balance, and functional independence.
• Support individuals following hospital discharge, helping them regain confidence and safely remain in their own homes.
• Carry out community visits independently and jointly with Reablement Workers, Occupational Therapists, and Social Workers.
• Assess mobility, transfers, gait, balance, and functional ability, adapting rehabilitation plans as individuals progress.
• Provide education, advice, and self-management strategies to service users, families, and carers.
• Work collaboratively within a multidisciplinary team to deliver coordinated, person-centred care.
• Contribute to discharge planning and support interventions that reduce hospital admissions and long-term care needs.
• Maintain accurate clinical records and ensure documentation complies with professional standards and local authority policies.
• Promote strengths-based practice, enabling service users to achieve meaningful rehabilitation goals and maximise independence.

About You

You will be a HCPC-registered Physiotherapist with experience in community rehabilitation, reablement, intermediate care, or discharge services. You will have excellent clinical reasoning skills and a passion for supporting adults to achieve greater independence through evidence-based rehabilitation.

You will be confident working autonomously within the community while collaborating effectively with multidisciplinary colleagues to deliver high-quality, person-centred care.

Requirements

• Degree or Diploma in Physiotherapy.
• Current HCPC registration.
• Experience working within community rehabilitation, reablement, intermediate care, or discharge services.
• Strong assessment, rehabilitation, and clinical reasoning skills.
• Experience working with adults following illness, injury, or hospital discharge.
• Excellent communication and multidisciplinary team-working skills.
• Ability to work independently within community settings and undertake home visits.
• Commitment to person-centred, strengths-based practice and continuous professional development.
