Reablement Officer

Tripod Partners are recruiting an experienced Reablement Officer for a locum opportunity with the London Borough of Newham. Working within East London Social Services, you will play a key role in delivering person-centred reablement services that support adults to regain independence, improve wellbeing, and remain safely within their own homes.
Working alongside the Service Manager, Senior Physiotherapist, Senior Occupational Therapist, and multidisciplinary colleagues, you will assess individuals’ reablement potential, develop personalised rehabilitation plans, monitor outcomes, and coordinate ongoing support. This role is ideal for a practitioner with experience in community-based reablement, case management, and integrated health and social care services.
Key Responsibilities
- Undertake comprehensive assessments to identify individuals’ reablement potential and ongoing support needs.
- Develop person-centred rehabilitation and support plans in partnership with service users, promoting independence and wellbeing.
- Establish agreed goals and measurable outcomes with individuals and regularly review progress.
- Monitor reablement programmes delivered by third-sector providers to ensure high-quality service delivery.
- Provide ongoing reviews and adapt reablement plans to maximise recovery and independence.
- Undertake social care assessments and light-touch reviews, maintaining case management responsibility throughout the reablement process.
- Provide or arrange appropriate equipment, assistive technology, and Telecare solutions to support independent living.
- Recommend ongoing support, signposting, and community services following discharge from the reablement service.
- Work collaboratively with health professionals, Adult Social Care teams, voluntary organisations, and other partner agencies to coordinate effective care.
- Maintain positive communication with colleagues, service users, carers, and external agencies to ensure continuity of care.
- Develop effective contingency plans to support safe and continuous service provision.
- Support the delivery of wider care services, including Telecare, where required.
- Assist the Service Manager and senior clinicians in the effective delivery and continuous improvement of the Reablement Service.
About You
You will have experience working within Adult Social Care, Reablement, community health, or rehabilitation services, with a strong commitment to delivering strengths-based, person-centred support. You will be confident carrying out assessments, managing cases, and working collaboratively across multidisciplinary teams.
You will possess excellent communication, organisational, and problem-solving skills, together with the ability to build positive relationships with service users, carers, and partner organisations to achieve meaningful outcomes.
Requirements
- Experience working within Adult Social Care, Reablement, rehabilitation, or community services.
- Knowledge of strengths-based practice and person-centred care planning.
- Experience undertaking assessments, reviews, and case management.
- Experience coordinating support with health, social care, and voluntary sector partners.
- Knowledge of equipment provision, assistive technology, and Telecare is desirable.
- Excellent communication, organisational, and record-keeping skills.
- Ability to manage a varied caseload and prioritise workload effectively.
- Commitment to promoting independence, recovery, and positive outcomes for adults living in the community.
