Lewisham and Greenwich NHS Trust is seeking an experienced Neuro Physiotherapist – Band 6 to join its Community Therapies service at Honor Oak Health Centre. This is an excellent opportunity for a skilled Neuro Physiotherapist with community experience to provide specialist assessment and rehabilitation to patients within a community setting.

The successful Physiotherapist will complete comprehensive neurological physiotherapy assessments, develop and implement individualised rehabilitation programmes and work collaboratively within a multidisciplinary team to support patients in achieving their rehabilitation goals.

Key Responsibilities

• Complete comprehensive neuro physiotherapy assessments for patients within the community setting.
• Identify individual rehabilitation needs and establish appropriate goals with patients.
• Create and implement individualised rehabilitation programmes based on assessment findings and patient goals.
• Provide appropriate neurological physiotherapy interventions to support functional recovery and independence.
• Monitor patient progress and adapt rehabilitation programmes as appropriate.
• Work collaboratively within the multidisciplinary team to coordinate patient care.
• Support patients to achieve agreed rehabilitation goals and positive outcomes.
• Maintain accurate and timely clinical documentation in line with professional and service requirements.

About You

We are looking for an experienced Band 6 Neuro Physiotherapist with previous community experience and strong neurological physiotherapy assessment and rehabilitation skills. You will be confident completing comprehensive assessments and developing goal-focused rehabilitation programmes.

You will have excellent communication and multidisciplinary working skills, with a patient-centred approach and a commitment to supporting individuals to achieve their rehabilitation goals within the community.

Requirements

• Relevant Physiotherapy qualification.
• Current HCPC registration.
• Relevant experience in neurological physiotherapy.
• Community Physiotherapy experience is essential.
• Experience completing comprehensive neuro physiotherapy assessments.
• Experience developing and implementing rehabilitation programmes.
• Ability to work effectively as part of a multidisciplinary team.
• Strong communication, clinical reasoning and organisational skills.
• Ability to work towards patient-centred rehabilitation goals.
