Job Details:
- Multiple Occupational Therapists required to support Reablement and Housing services.
- Carry out occupational therapy assessments for adults with physical disabilities and complex needs.
- Support individuals to maximise independence and remain safely in their homes.
- Assess for equipment, environmental adaptations and housing solutions where required.
- Develop and implement person-centred intervention plans.
- Work collaboratively with multidisciplinary teams including social workers, housing teams and health professionals.
- Maintain accurate case records and documentation in line with service standards.
Skills / Experience:
- Qualified Occupational Therapist with HCPC registration.
- Experience working within adult social care.
- Experience in reablement and/or housing adaptations.
- Ability to manage a caseload independently.
- Strong communication and multidisciplinary working skills.
- Full UK driving licence and access to a vehicle (essential).
Working Hours: Full time or part time considered.
