Occupational Therapist – Reablement Team

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity within Wiltshire Social Services. Based at Monkton Park Council Offices, this rewarding role is within the Reablement Team, supporting adults to regain confidence, rebuild daily living skills, and maximise their independence following a hospital stay.

Working within a supportive multidisciplinary team, you will deliver strengths-based, person-centred Occupational Therapy interventions that empower individuals to achieve meaningful goals and remain independent in their own homes. This is an excellent opportunity to make a genuine impact while contributing to the ongoing development of a high-performing reablement service.

Key Responsibilities

  • Complete comprehensive Occupational Therapy assessments for adults following hospital discharge.
  • Develop and implement tailored, strengths-based reablement plans that promote independence and recovery.
  • Support individuals to regain confidence and functional skills in activities of daily living.
  • Use clinical reasoning to identify appropriate interventions, equipment, and assistive solutions.
  • Provide specialist advice on equipment provision, safe practice, and risk management.
  • Monitor progress and adapt intervention plans to reflect changing needs and goals.
  • Work collaboratively with service users, carers, health professionals, and multidisciplinary colleagues to achieve positive outcomes.
  • Act as a trusted clinical advisor within the Reablement Team, supporting colleagues with professional guidance and decision-making.
  • Maintain accurate, timely, and legally compliant clinical records.
  • Contribute to continuous service improvement initiatives and the development of high-quality reablement practice.

About You

You will be an experienced Occupational Therapist with a passion for supporting adults to regain independence through person-centred, strengths-based practice. You will be confident managing community-based caseloads, delivering high-quality assessments, and working collaboratively within a multidisciplinary environment.

You will possess excellent clinical reasoning, communication, and organisational skills, together with the ability to motivate individuals and support colleagues through your professional expertise.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience working within Adult Social Care, Community Occupational Therapy, or Reablement Services.
  • Experience completing functional assessments and developing person-centred reablement plans.
  • Knowledge of equipment provision, assistive technology, and safe moving and handling practices.
  • Strong clinical reasoning, assessment, and report-writing skills.
  • Excellent communication and multidisciplinary team-working abilities.
  • Ability to work independently while contributing positively to service improvement initiatives.
