Job Overview
Tripod Partners are currently recruiting for an experienced Band 6 Occupational Therapist to join the Intensive Rehab Service at NELFT. This is an ongoing locum opportunity based at Grays Court Community Hospital and Goodmayes Hospital, supporting patients across a community rehabilitation setting.
The successful Occupational Therapist will be responsible for the independent assessment, treatment and discharge planning of patients within the community rehabilitation team. You will manage your own caseload, use recognised outcome measures and apply clinical reasoning to develop and evaluate appropriate treatment plans.
Key Responsibilities
- Undertake independent Occupational Therapy assessments of patients within a community rehabilitation setting.
- Plan, deliver and evaluate appropriate treatment programmes based on individual patient needs.
- Manage and prioritise your own caseload effectively according to clinical need and risk.
- Complete discharge planning and support patients to transition safely from the service.
- Use recognised outcome measures and clinical reasoning to guide assessment, intervention and treatment planning.
- Monitor patients’ progress and identify individuals who are deteriorating or at risk of deterioration.
- Support patients transitioning into the community rehabilitation service following discharge from acute hospital settings.
- Work collaboratively with Physiotherapists, Rehabilitation Assistants and the wider multidisciplinary team.
- Ensure accurate and timely clinical documentation in accordance with HCPC standards and relevant policies.
- Provide appropriate advice and rehabilitation to promote independence and safe discharge.
- Start the working day in the office before completing community-based visits.
- Work in an agile manner when completing community visits and finish the working day with patients as required.
About You
We are looking for a confident and experienced Band 6 Occupational Therapist with previous NHS experience and a strong interest in community rehabilitation. You will be confident managing your own caseload and making independent clinical decisions regarding assessment, treatment and discharge planning.
Experience of working in an acute hospital environment would be particularly beneficial, as many patients will be transitioning into the service following an acute hospital admission. This experience will support your ability to recognise patients who are deteriorating or at risk of deterioration.
Requirements
- Qualified Occupational Therapist with relevant professional registration.
- Previous post-qualification Occupational Therapy experience within the NHS.
- Experience within a community rehabilitation setting is preferred.
- Ability to independently assess, treat and discharge patients.
- Experience managing and prioritising an independent caseload.
- Strong clinical reasoning and treatment-planning skills.
- Ability to use recognised outcome measures to assess and demonstrate patient progress.
- Experience of acute hospital patient management would be advantageous.
- Ability to recognise and appropriately respond to deteriorating or at-risk patients.
- Strong understanding of HCPC standards and accurate clinical documentation.
- Good communication and multidisciplinary working skills.
- Car driver preferred but not essential due to the community-based nature of the role.
- Flexibility to work within the service’s operating hours, currently between 08:00 and 18:00.
