Tripod Partners are recruiting a Band 6 Community Occupational Therapist for a locum role within
East Riding of Yorkshire NHS. This position focuses on delivering occupational therapy support within
community services, helping individuals maintain independence and manage daily activities following illness, injury, or
changes in physical function.
Working as part of a community multidisciplinary team, the Occupational Therapist will assess patients in their own homes
and local settings, with particular focus on individuals recovering from stroke. The role involves
providing rehabilitation support, identifying functional needs, and recommending appropriate interventions to improve
independence and quality of life.
Key Responsibilities
- Carry out occupational therapy assessments within community settings.
- Support individuals recovering from stroke through rehabilitation and functional interventions.
- Develop person-centred treatment plans that promote independence and safe daily living.
- Recommend appropriate equipment or adaptations to support independence at home.
- Work collaboratively with multidisciplinary teams delivering community rehabilitation services.
- Maintain accurate clinical documentation and contribute to ongoing care planning.
- Support patients and carers with advice and guidance regarding rehabilitation and independent living.
About You
You will be a qualified Occupational Therapist with experience working within community rehabilitation
services. Experience supporting individuals following stroke will be beneficial, particularly where therapy interventions
focus on restoring independence and improving daily functioning.
You will be confident working independently within community settings and collaborating with wider health and social care
teams to deliver effective rehabilitation outcomes.
Requirements
- Recognised qualification in Occupational Therapy.
- Registration with the Health and Care Professions Council (HCPC).
- Community rehabilitation experience, particularly with stroke patients.
- Degree and post-graduate experience of approximately 3 years desirable but not essential.
- Experience using SystmOne desirable but not essential.
- Full driving licence and access to own car required due to the community nature of the role.
