Brief Job Overview

Tripod Partners are currently recruiting for a qualified Occupational Therapist to join the Reablement service at Southampton City Council. This locum opportunity involves supporting individuals who are currently receiving bridging support while awaiting the commencement of Reablement services.

You will undertake Occupational Therapy assessments to identify opportunities to maximise independence, reduce ongoing care needs and support timely progression into Reablement. The role involves making recommendations around equipment, adaptations, moving and handling and rehabilitation interventions, while working closely with Reablement staff, the wider therapy team and partner agencies.

Key Responsibilities

• Undertake Occupational Therapy assessments for individuals receiving bridging support while awaiting Reablement.
• Identify opportunities to maximise independence and reduce ongoing care needs.
• Support timely progression into appropriate Reablement services.
• Provide professional recommendations regarding equipment and adaptations.
• Assess and advise on moving and handling requirements to support safe and effective care.
• Recommend appropriate rehabilitation interventions based on individual needs and desired outcomes.
• Work collaboratively with the wider therapy team, Reablement staff and partner agencies.
• Provide person-centred and outcome-focused Occupational Therapy support.
• Maintain accurate and comprehensive records in accordance with professional and organisational standards.
• Contribute to care planning, risk assessment and review processes.

About You

We are looking for a qualified Occupational Therapist with current HCPC registration and experience working within health or social care. Experience within Reablement or community-based services would be particularly relevant to this role.

You will be confident undertaking assessments and making professional recommendations around equipment, adaptations, moving and handling and rehabilitation. Strong communication and partnership-working skills are essential, together with a commitment to promoting independence and achieving person-centred outcomes.

Requirements

• Qualified Occupational Therapist.
• Current registration with the Health and Care Professions Council (HCPC).
• Experience working within health or social care settings.
• Experience within Reablement or community-based services is desirable.
• Experience undertaking Occupational Therapy assessments.
• Ability to make recommendations regarding equipment, adaptations, moving and handling and rehabilitation interventions.
• Ability to work collaboratively with therapy, Reablement and partner agency colleagues.
• Full UK driving licence and access to a vehicle is essential.
