Brief Job Overview

Tripod Partners are currently recruiting for a qualified Occupational Therapist to join Southampton City Council at Sembal House, within Hampshire Social Services. This is an ongoing locum opportunity supporting individuals accessing a Reablement Service.

The successful Occupational Therapist will assess individuals currently receiving bridging support while they await commencement of Reablement. The role will focus on identifying opportunities to maximise independence, reduce ongoing care needs and support timely progression into appropriate Reablement services.

Key Responsibilities

• Undertake Occupational Therapy assessments for individuals currently receiving bridging support and awaiting commencement of Reablement.
• Identify opportunities to maximise independence and reduce ongoing care and support needs.
• Support the timely progression of individuals into Reablement services.
• Provide recommendations regarding appropriate equipment and adaptations to support independence and safety.
• Provide advice relating to moving and handling requirements and rehabilitation interventions.
• Work collaboratively with the wider therapy team, Reablement staff and partner agencies.
• Deliver person-centred and outcome-focused Occupational Therapy support.
• Contribute to care planning, risk assessment and review processes.
• Maintain accurate and timely professional records in accordance with professional and organisational standards.

About You

We are looking for a confident and qualified Occupational Therapist with experience working within a health or social care environment. Experience within Reablement or community-based services would be particularly beneficial.

You should have strong assessment and clinical reasoning skills, with the ability to identify practical interventions that promote independence and support effective rehabilitation. Excellent communication and partnership-working skills are essential, alongside the ability to work collaboratively with therapy and Reablement colleagues.

Requirements

• Relevant Occupational Therapy qualification.
• Current HCPC registration.
• Experience working within a health or social care setting.
• Experience within Reablement or community-based services is desirable.
• Ability to undertake Occupational Therapy assessments and identify appropriate interventions.
• Knowledge of equipment, adaptations, moving and handling and rehabilitation interventions.
• Strong communication, assessment and professional record-keeping skills.
• Full driving licence and access to a vehicle is essential.
