Undertake statutory assessments, care planning, and safeguarding enquiries in line with the Care Act 2014, Mental Capacity Act 2005, and relevant legislation to promote independence and wellbeing.

Manage a diverse caseload including adults with physical disabilities, mental health issues, learning disabilities, and older adults requiring support.

Collaborate closely with service users, families, carers, health professionals, and community partners to deliver integrated, strengths-based care and support plans.

Use a holistic and outcome-focused approach to empower individuals to make informed choices and maintain independence within their communities.
