Job details: Occupational Therapist required to support a Double to Single Handed Care project, reviewing existing care packages and identifying opportunities to maximise independence and ensure safe, proportionate care provision.

- Review double handed care packages with a focus on safe reduction to single handed care where appropriate
- Undertake functional assessments within service users’ homes
- Complete moving and handling assessments and recommendations
- Recommend appropriate equipment and minor adaptations to support independence
- Work collaboratively within a multidisciplinary team to promote strengths-based practice
Essential skills/experience:

- Qualified Occupational Therapist with HCPC registration
- Experience within Adult Social Care
- Strong adaptations experience
- Experience reviewing care packages and promoting independence
