Job details:
Band 6 Occupational Therapist required to join the Community Rehabilitation Services Stroke Team.
This is an urgent requirement to start as soon as possible for an initial 2-month period.
The post holder will provide specialist Occupational Therapy assessments and interventions in a community setting
to adults with a diagnosis of stroke, working within a multidisciplinary team to deliver goal-focused rehabilitation.
- Provide specialist Occupational Therapy assessment and treatment for stroke patients in the community
- Manage a community-based caseload, delivering goal-focused neuro rehabilitation
- Work collaboratively within a multidisciplinary team to support recovery and independence
- Develop, implement and review individualised rehabilitation plans
- Contribute to effective discharge planning and ongoing community support
Essential skills & experience:
- HCPC registration
- Neuro rehabilitation experience
- Experience working in a community setting
Desirable skills:
- Further experience within community rehabilitation services
Working hours: 30 hours per week (4 days). Monday to Friday, 08.30 – 16.30. Days flexible.
