Community Physiotherapist – Band 6
Job Overview
We are recruiting an experienced Band 6 Community Physiotherapist to join Hywel Dda University Health Board at Withybush General Hospital in Pembrokeshire. This locum position offers a varied community caseload, supporting patients across Pembrokeshire who are referred through the Community Physiotherapy Service.
Working within the South West Wales NHS network, you’ll deliver person-centred, evidence-based physiotherapy to help patients maximise independence, mobility, and quality of life in their own homes and community settings.
Key Responsibilities
– Manage a mixed community caseload, assessing, treating, and reviewing patients referred via the community physiotherapy waiting list.
– Conduct holistic, person-centred assessments and develop tailored treatment and rehabilitation programmes.
– Deliver physiotherapy interventions in patients’ homes or community settings to promote functional recovery and prevent hospital admissions.
– Work collaboratively within a multidisciplinary team to ensure coordinated and effective care.
– Educate patients, carers, and families on safe self-management strategies.
– Maintain accurate and timely clinical records in line with NHS and HCPC standards.
– Contribute to service development and quality improvement within the community therapy team.
About You
You’ll be a compassionate and adaptable Physiotherapist with strong experience in community settings. You’ll have the ability to work autonomously while maintaining close collaboration with multidisciplinary colleagues to deliver effective, patient-focused rehabilitation and support.
Requirements
– Degree or Diploma in Physiotherapy (or equivalent qualification)
– Current registration with the Health and Care Professions Council (HCPC)
– Proven experience working in a community physiotherapy setting (essential)
– Excellent assessment, clinical reasoning, and time management skills
– Strong communication and interpersonal abilities
– Car driver with access to a vehicle for work purposes (essential)
– Commitment to patient-centred, evidence-based care
Contract & Salary
– Contract type: Locum
– Contract length: Ongoing (until January 2026)
– Working hours: 37.5 hours per week, Monday to Friday, 08:30 – 16:30
– Location: Hywel Dda University Health Board – Withybush General Hospital, Pembrokeshire
– Salary: £32 per hour
