Service: Early Intervention in Psychosis (EIP)

 

Job Details:

 

  • 4x Band 6 Care Coordinators required to start ASAP within the Early Intervention in Psychosis Service.
  • Provide high-quality care coordination to service users aged 16–65 experiencing or recovering from a first episode of psychosis.
  • Undertake specialist assessments and support individuals through the 3-year EIP pathway in line with NICE guidelines.
  • Deliver recovery-focused, evidence-based interventions in both community and inpatient settings.
  • Develop, implement and review person-centred care plans within a recovery framework.
  • Work within a multidisciplinary team using integrated health and social care approaches.
  • Undertake Connected Lives assessments to identify social care outcomes.
  • Participate in Family Intervention training and deliver family interventions under supervision.
  • Maintain accurate clinical documentation in line with governance and professional standards.

 

Skills / Experience:

 

  • Minimum 1 year experience in Community Mental Health.
  • Experience working with individuals experiencing psychosis or severe mental health conditions.
  • Ability to assess, plan, implement and review care effectively.
  • Strong understanding of recovery-oriented practice.
  • Excellent communication and multidisciplinary working skills.
  • Full UK driving licence and access to a car (essential).
  • Ability to work on-site (remote working not available).

 

Working Hours: 37.5 hours per week, Monday to Friday, 09:00 – 17:00.
