Job details:  Intensive Rehab & Therapy Svc. Previous community and discharge planning experience • Car driver and access to own car for work preferred (non-car drivers would also be considered) • Community based rehabilitation working on a 6 week pathway • Caseload consists of patients on the Discharge-To-Assess (D2A) pathway that currently require new 24hour care and have potential to progress to having less dependent needs / being discharged home. • Lead on discharge planning with an MDT approach, working closely with Physiotherapists, Rehabilitation Assistants, Social Workers and Nursing Home staff. Commonly encountered conditions include: • Deconditioning from acute hospital • Deconditioning post infection such as UTI, RTI • Falls • Rehabilitation of orthopaedic patients (eg. Post # NOF) • PD • COPD. Working hours: shifts, 08.30 – 16.30. Monday to Friday. Up to 37.5 hours per week.
