Job details:  Occupational Therapist required to:
Deliver a high standard OT service in designated clinical areas.
Manage a defined complex caseload under the management of the Team Leader, using evidence based, client centred principles to assess, plan and evaluate OT interventions, ensuring compliance with HCPC, RCOT, CQC and Trust clinical and quality standards.
To ensure as far as is reasonably practical the safety of the patients in your care.
To plan and organise a designated caseload of complex patients within the designated service and own professional activities under the management of Team Leader.
To work with the patient and carers to identify and meet OT goals, using standardised and OT assessment tools to assess their skills deficits, occupational needs and achieve functional fitness for timely discharge.
In collaboration with the patient, to plan and implement single or a programme of interventions using treatment techniques to meet the patient’s needs re; timely discharge.  These will be achieved using professional and theoretical knowledge, and specialist clinical reasoning skills.
To monitor, evaluate and modify treatment interventions to measure progress and ensure effectiveness, involving patient, carer and MDT as appropriate.
To ensure Therapy goals are integral to the multidisciplinary care plan re: safe and timely discharge.
To apply an enhanced understanding of effects and impact of illness, frailty and disability on patient’s abilities and provide advice and education regarding lifestyle changes / adaptations to their physical and social environment.
To use frequent periods of light / moderate physical effort to carry out interventions with patients depending on their health status and needs.
To undertake all aspects of clinical duties, as an autonomous practitioner.
To be professionally and legally accountable for all aspects of your own work, including the management of patients in your care. 
To respect the individuality, values, cultural and religious diversity of patients and carers and contribute to the provision of a service that is sensitive to these needs.
To be responsible for the safe and competent use of equipment and patient appliances and aids by patients and by junior and student therapists, through teaching, training and supervision of practice.
Experience:

• Experience of multi-disciplinary working
• Comprehensive range of junior rotations with experience of complex caseload management.
• Experience of facilitating discharges from the acute setting and/or managing a caseload of patients within the community.
• In-service training and reflective learning
• Ability to work as an autonomous practitioner
• Prioritisation of workload

Qualifications:

• BSc / MSc Occupational Therapy
• HCPC Registered Occupational Therapist

Knowledge:

• Broad knowledge of; respiratory, neurology, elderly and trauma and orthopaedic conditions and their management.
• Principles and practices of rehabilitation / reablement
• Sound clinical reasoning
• Literature search/review
• Current evidence-based practice
• Detailed anatomy and physiology relevant to the speciality
• Health and Safety and risk awareness

Job Title:  Physiotherapist – Band 6 (Part time)

Job details:  Physiotherapist required to start asap for 12 weeks. Inpatient Therapy. Working on a single ward 19-20 bedded inpatient rehabilitation unit. To complete initial assessments and devise appropriate therapy and discharge plans for patients presenting with a wide variety of conditions. Mainly 65 years and older. • The post holder will be working as part of the inpatient therapy team with a focus on rehabilitation. This will involve helping patients to engage as independently as possible in activities which enhance their health & wellbeing.

Interventions will be carried out within patients’ own homes, 24-hour care facilities and clinics. 
• The post holder will work as part of the integrated team to plan, deliver and evaluate all aspects of the patient treatment process. This at times will include onward referrals to appropriate community-based health and social care agencies.

They will assess and recommend changes to packages of care. They will facilitate safe hospital discharges but providing timely home visits and equipment orders as needed. To have experience of supporting junior staff in developing skills. To be able to triage complex needs and use current team pathways. They will have extensive social services OT experience.
