Job details:  Community Recovery. To work as an active member of the integrated multi-disciplinary team and promote effective multi-agency partnership working. To provide Occupational Therapy assessment and interventions. Provide a range of specialist Occupational Therapy assessments and 1:1/group interventions for service users with complex mental health needs. Develop support plans and reports that promote social inclusion, wellbeing, and recovery. Care co-ordination – ensure continuous responsibility for coordinating the care of an identified group of service users as part of a team. To carry out generic mental health assessments. Using assessment outcomes to make clinical formulations, and devise treatment plans. Promoting inter-agency collaboration around the social inclusion agenda locally, establishing recovery focused group programme and promoting the active rehabilitation and recovery for users of mental health services. Act as a role model for Occupational Therapy, raising the profile and value of the role within the service. Support the mental health transformation project. Skills/experience: Qualified Occupational Therapist. Experience of acute or community mental health services. Experience of working in/with an MDT. Mental health recovery focused. Caseworker/care co-ordination. Understanding of OT processes, models, and evidence-based practice. Groupwork facilitator. Knowledge of equipment and adaptations desirable. Qualified SALT and relevant registration required, HCPC.
Completion of RCSLT dysphagia competencies
Specialist knowledge of assessment tools and therapeutic interventions relevant to acquired and progressive neurological and non-neurological speech and language disorders in adults
Specialist knowledge of assessment tools and therapeutic interventions relevant to acquired and progressive neurological and non-neurological swallowing disorders in adults
Experience of working in a Community Setting.
Competent in use of SystmOne & other IT systems.
Working hours: Monday to Friday, 09.00 – 17.00. Full time but will consider part time hours.
