The Reviewing Officer plays a vital role within Adult Social Care hospital teams, ensuring timely and appropriate transitions from hospital to home or alternative care settings. This requires a clear understanding of individual needs, existing support networks, and available community resources.

 

Key Responsibilities

 

  • Assessment and Reassessment: Carry out comprehensive assessments of individuals’ social care needs on admission and at key stages of their hospital stay, including physical health, mental health, cognition, daily living skills and social circumstances.
  • Care Planning and Coordination: Develop personalised care plans in partnership with individuals, families, hospital staff and other professionals, identifying appropriate care options such as home care, residential care or rehabilitation services.
  • Discharge Planning: Lead or contribute to discharge planning meetings to ensure safe and timely discharge, arranging equipment, adaptations and ongoing care services as required.
  • Safeguarding: Identify and respond to safeguarding concerns, working with adult safeguarding teams and statutory bodies to protect individuals’ safety and wellbeing.
  • Advocacy and Support: Act as an advocate to ensure individuals’ views and preferences are heard, providing information and guidance to individuals and families navigating the social care system.
  • Liaison and Partnership Working: Build and maintain effective working relationships with hospital teams, community health services, local authorities and third-sector organisations to support integrated care.
  • Risk Management: Identify and mitigate risks associated with discharge, including falls, re-admission and insufficient support at home.
