Lewisham and Greenwich NHS Trust is seeking a Band 6 Community Physiotherapist to join the community service based at Honor Oak Health Centre. This role focuses on delivering comprehensive physiotherapy assessment and treatment to patients within the community, supporting recovery, independence, and effective management of a wide range of clinical needs.
Key Responsibilities

- Carry out comprehensive physiotherapy assessments and deliver treatment to community-based patients.
- Manage a varied caseload including orthopaedic, elderly, respiratory, and general community cases.
- Develop and implement individualised treatment plans based on clinical reasoning.
- Support rehabilitation, functional recovery, and independence.
- Work collaboratively with multidisciplinary and community teams.
- Maintain accurate and timely clinical documentation in line with NHS standards.
About You
You will be an experienced Physiotherapist with a strong background in community practice. You will be confident working independently, managing your own caseload, and delivering high-quality rehabilitation in patients’ homes and community settings.
Requirements

- Degree or equivalent qualification in Physiotherapy.
- Current HCPC registration.
- Experience completing comprehensive community physiotherapy assessments.
- Clinical experience in orthopaedics, elderly care, respiratory, and community rehabilitation.
- Strong communication, organisational, and time-management skills.
Contract & Working Pattern
- Degree or equivalent qualification in Physiotherapy.

- 37.5 hours per week
- Monday to Friday
- 08:30 – 16:30
- Competitive rate – £negotiable
