An opportunity has arisen for an Independent Living Assessor to join the busy Information and Advice (Front Door) Team at the Royal Borough of Kensington and Chelsea. This role is ideal for a proactive practitioner who thrives in a fast-paced environment and is confident managing high volumes of enquiries and referrals.
You will support Adult Social Care services by screening referrals, responding to safeguarding concerns, and promoting independence and wellbeing for residents.
Key Responsibilities
- Answer incoming calls and manage a range of social care queries.
- Screen and process Adult Social Care referrals and safeguarding concerns.
- Provide appropriate information, advice, and signposting to residents and professionals.
- Use Mosaic to accurately document case actions and assessments.
- Support Social Work and Occupational Therapy teams in arranging services, equipment, and minor adaptations.
- Promote independence and wellbeing for residents across the borough.
About You
You will be an enthusiastic and organised practitioner with experience in Adult Social Care front door or screening services. You will be confident handling sensitive information, making decisions under pressure, and working as part of a multidisciplinary team.
Requirements
- Experience working within Adult Social Care or a similar front door / screening function.
- Knowledge of safeguarding processes.
- Experience using Mosaic or similar case management systems.
- Strong communication and customer service skills.
- Ability to manage a busy and fast-paced workload.
Working Pattern
- Office-based for approximately 50% of the working week
- Part of a busy front door service environment
