Key Responsibilities:

Undertake Care Act assessments and reviews for older adults, developing tailored care plans that reflect individual needs, risks, and desired outcomes.

Manage safeguarding concerns and complete Section 42 enquiries in line with statutory adult protection procedures.

Support hospital discharge planning and community-based interventions, promoting independence and reducing the need for long-term care placements.

Collaborate with families, carers, NHS professionals, and voluntary sector partners to deliver holistic, integrated care and improve the well-being of older adults in the community.
