We are seeking a dedicated and compassionate Personal Advisor to join our Leaving Care Team, working with young people aged 16–25. This role involves providing emotional and practical support to ensure care-experienced young people achieve stability, well-being, and success in all areas of life.
Key Responsibilities:

Build positive, trusting relationships: Act as a consistent point of contact and advocate for care leavers, offering guidance and encouragement.

Support pathway planning: Contribute to and review Pathway Plans that reflect the young person’s goals around education, housing, employment, and health.

Promote independence: Help young people access and sustain suitable accommodation, manage finances, and develop life skills.

Coordinate multi-agency support: Liaise with social workers, housing officers, education providers, and health professionals to ensure integrated care and opportunities.
