Key Responsibilities:

Assist in early discharge planning and community rehabilitation for inpatients and community-based patients.

Support physiotherapists in implementing care plans and rehabilitation programmes.

Provide and review equipment and assist with the management of care packages.

Work collaboratively with the multidisciplinary team to ensure continuity of care.

Maintain accurate documentation in line with organisational and professional standards.
Skills and Experience Required:
-
Experience in early discharge and community rehabilitation.
-
Knowledge of equipment provision and care package review.
-
Driving licence desirable to support travel for community visits.
Working Hours: 37.5 hours per week, Monday to Friday.
