Undertake statutory assessments, support planning, and safeguarding interventions in line with the Care Act 2014, Mental Capacity Act 2005, and relevant local procedures.

Manage a diverse caseload, including individuals with complex needs such as learning disabilities, mental health challenges, physical disabilities, and older people.

Collaborate with families, carers, health professionals, and other agencies to provide coordinated, multi-disciplinary support tailored to each individual’s strengths and goals.

Apply a strengths-based, outcomes-focused approach, supporting people to make informed choices and achieve sustainable solutions.
