Key Responsibilities:

Undertake strength-based assessments and reviews in line with the Care Act 2014, ensuring person-centred support planning.

Manage a diverse caseload involving adults with physical disabilities, learning disabilities, mental health needs, and older people.

Work collaboratively with service users, families, carers, and multi-agency partners to promote independence and wellbeing.

Apply safeguarding procedures effectively to respond to concerns and protect adults at risk.

Maintain accurate and timely case recording, ensuring legal and procedural compliance at all times.
