View JobSefton Borough Council is seeking a dedicated and person-centred Adult Social Worker to join our Adult Social Care team. This role supports adults with care and support needs, including older people, those with physical or learning disabilities, and people experiencing mental ill health. You will work in partnership with individuals, carers, families, and professionals to promote independence, dignity, and well-being in line with the Care Act 2014.
Key Responsibilities:

Undertake strengths-based assessments and reviews to determine eligible needs and develop care and support plans.

Apply safeguarding procedures to protect adults at risk, including conducting Section 42 enquiries where required.

Work closely with NHS, housing, and community partners to deliver integrated and coordinated care.

Maintain clear and accurate records in accordance with Sefton Council’s policies and national legislation.
