In this role, you will work with adults who have a variety of care and support needs, including older adults, individuals with physical disabilities, and those experiencing mental health challenges. Your focus will be on delivering person-centred assessments and interventions that promote independence, dignity, and wellbeing within the community.
Key Responsibilities:

Carry out Care Act 2014 assessments and reviews to identify and respond to adults’ social care needs.

Undertake safeguarding enquiries under Section 42 of the Care Act, ensuring protection of vulnerable adults.

Collaborate with health, housing, and voluntary sector partners to coordinate care and support services.

Apply the Mental Capacity Act 2005 to assess decision-making capacity and support best interest decisions.

Maintain accurate case records and reports in line with statutory requirements and Harrow Council policies.
