Job details: Community Therapies – Intermediate Care (Falls, Frailty, Rehabilitation). JD available on request.

- Undertake holistic OT assessment, clinical diagnosis and individualised treatment plans for patients with complex and/or chronic presentations, including palliative patients
- Contribute to effective inter-disciplinary working, providing a high-quality day to day service to the active caseload
- Provide supervision, support and line management to junior staff, and mentorship to students
- Support case management by providing expert clinical advice and therapy to community-based patients at risk of deterioration or avoidable hospital admission
- Work with senior staff to develop and enhance the service in partnership with Integrated Care Teams and relevant stakeholders
- Participate in a rota within Intermediate Care covering 08:00–20:00, 7 days a week across service streams
