Working collaboratively with service users, families, carers and multidisciplinary teams, you will develop and deliver personalised programmes tailored to meet individual needs.

Carry out comprehensive assessments of functional abilities and daily living requirements.

Develop, implement and review individualised intervention plans to promote independence and quality of life.

Work closely with service users, carers and professionals to coordinate effective care and support.

Maintain accurate and up-to-date case records in line with council and professional standards.

Provide advice, guidance and training to service users, carers and colleagues on adaptive strategies and equipment.

Contribute to service development initiatives to improve occupational therapy services.
