Undertake comprehensive assessments of functional abilities and rehabilitation needs in the community.

Plan, implement and review evidence-based interventions to support independence and quality of life.

Work closely with service users, families, carers and other professionals to coordinate personalised care plans.

Maintain accurate and timely records in line with council and professional standards.

Provide guidance, support and training to service users and carers on adaptive techniques and equipment use.

Contribute to service improvement and the development of community-based rehabilitation services.
