Key Responsibilities:

Provide high-quality, evidence-based occupational therapy assessments and interventions across community settings, including patients’ homes, clinics, and rehabilitation units.

Work collaboratively within a multi-disciplinary team, including physiotherapists, nurses, social workers, and other healthcare professionals, to ensure holistic patient care.

Develop and implement personalised treatment plans to support patients in achieving their functional goals and improving their quality of life.

Contribute to safe and timely discharge planning by promoting independence in daily living activities and reducing the risk of hospital readmission.

Maintain accurate and timely clinical documentation in line with Trust policies and professional standards.

Participate in ongoing professional development, supervision, and training opportunities, and contribute to service improvement initiatives.
