Carry out statutory duties under the Care Act 2014, Mental Capacity Act 2005, and safeguarding legislation, ensuring the protection, empowerment, and wellbeing of vulnerable adults.

Manage a diverse caseload including care assessments, support planning, safeguarding enquiries, and hospital discharges, using a strengths-based and outcome-focused approach.

Work collaboratively with service users, families, carers, health professionals, and partner agencies to deliver integrated and coordinated care.
