- Allocated Occupational Therapist for adults and children with physical disabilities and learning disabilities, providing preventative support to promote independence
- Responsible for assessing, developing, reviewing and monitoring individual care plans relating to adaptations, equipment, and moving and handling to support independent living
- Work within a strengths-based model, focusing on prevention, promoting independence, and identifying solutions to enable citizens to remain in their own homes
- Commission services for the provision of adaptations and equipment, enabling citizens and carers to achieve their full potential for independent living
- Liaise with voluntary and statutory organisations at local and national level
- Act as Duty Officer, providing consultation to internal care management teams, external health and social care professionals, and members of the public regarding disability, equipment solutions, and moving and handling
- Screen referrals into the Occupational Therapy service
