Undertake statutory assessments, care and support planning, safeguarding enquiries, and Mental Capacity Act assessments in line with the Care Act 2014 and other key legislation.

Manage a diverse caseload including older adults, individuals with physical disabilities, mental health needs, and learning disabilities, using a strengths-based, person-centred approach.

Work collaboratively with service users, carers, families, and professionals from health, housing, and voluntary sectors to deliver integrated, outcome-focused care.

Promote independence and wellbeing by identifying preventative support and enabling people to access services that help maintain their quality of life.
