Carry out strengths-based assessments and personalised care planning in line with the Care Act 2014, focusing on individual outcomes and wellbeing.

Manage a diverse caseload including older adults, people with learning or physical disabilities, and individuals with mental health needs.

Work collaboratively with health professionals, housing providers, voluntary sector partners, and families to deliver coordinated, multi-agency support.

Lead and respond to safeguarding adults enquiries (Section 42), ensuring interventions are legally compliant and person-centred.
