Undertake statutory responsibilities under the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults framework, ensuring safety, dignity, and independence.

Manage a diverse caseload including complex assessments, care planning, safeguarding enquiries, and hospital discharges, working to deliver proportionate and effective interventions.

Collaborate with individuals, carers, health professionals, and community partners to develop coordinated, multi-agency care solutions.
