Undertake statutory responsibilities under the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults framework, ensuring safety, dignity, and independence.

Manage a diverse caseload including complex assessments, care planning, safeguarding enquiries, and hospital discharges, working to deliver proportionate and effective interventions.
