Carry out statutory duties under the Care Act 2014, including strengths-based assessments, care planning, safeguarding enquiries, and Mental Capacity Act assessments.

Manage a varied caseload involving older people, adults with disabilities, and those with mental health conditions, ensuring high-quality, timely interventions.

Deliver person-centred, outcome-focused support that promotes dignity, autonomy, and long-term wellbeing.
