Fulfil statutory duties under the Care Act 2014, including assessments, care and support planning, safeguarding investigations, and Mental Capacity Act responsibilities.

Manage a varied caseload, working with older people, adults with physical and learning disabilities, and those with mental health needs or complex vulnerabilities.

Apply a strengths-based and person-centred approach, promoting choice, control, and positive outcomes for service users and their carers.

Collaborate with health, housing, voluntary and community partners to deliver coordinated, multi-agency support that meets individual needs.
